This information is intended to highlight the various safety information regarding Lifting and Hoisting. The expectation is that all field personnel will be trained in the requirements prior to accepting a field assignment as mandated by the Upstream Americas Wells Leadership Team.

The attached document, Equipment Shipping Check List – 2-7-2017 may be used to assist in determining equipment readiness.

What is the required information on sling tags?Sling manufacturer:

Is it required to proof test general use wire rope slings annually?No, general use wire rope slings require an initial proof test, then an annual inspection by a qualified person. A color code or updated tag is required to indicate inspection date and documented in the Lifting and Hoisting registry.
What classifies a pre slung sling?Pre-slung slings are integral to pieces of equipment shipped to and from locations. Any sling that is left on the load and not stored properly is classified as pre slung and required to have an annual recertification.
When can we use synthetic slings?Synthetic Slings can be used where the application of wire rope will damage or not be capable of handling the load.
When can we use Alloy chain slings?Alloy chain slings can be used when approved by the lift sponsor; rated and certified for the intended use.
Is it a requirement that an anchor bolt shackle (4 part) be used for all Lifting and Hoisting activities?No, anchor bolt shackles (4 part) must be used when left at heights. Screw pin shackles (2 part) can be used for temporary lifting connections.
What lifting equipment training is required to work on a Shell location?Shell requires companies to have a competency assurance process in place that can be audited.
Can designated signal person (DSP) also act as a load handler for mobile crane activities?No, the designated signal person (DSP) and the load handler need to be separate roles.
When a wire line lubricator is resting on the well head, is that considered a suspended load? No, crane operations involving coil tubing, slick line or lubricators where the load is resting on the well head is not considered a suspended load.
Does Shell have a minimum requirement for outrigger pad size?Yes. Outrigger pads must be at least 3.5 times the OEM supplied floats.
Are there any requirements for barricading the lift area? Yes, the superstructure of the crane must always be barricaded. The lift area must be controlled to prevent unauthorized persons from entering, in some cases a physical barricade will be required.
When is it required to restrict crane capacity?For critical lifts, the load must not exceed 75% of the rated crane capacity. For complex lifts involving personnel, the load must not exceed 50% of the rated crane capacity, and anytime the OEM requires capacity restrictions due to environmental conditions.
What is a powered industrial truck (PIT)?Powered industrial truck (PIT) is a front end loader/telehandler (zoom boom) with lifting attachments (stinger, pallet fork, grapple fork, etc) or high mast forklift.
What are the requirements for (PIT) lifting attachments?Powered industrial truck (PIT) attachments must be approved for use in writing by the (PIT) manufacturer, not the attachment manufacture or rental company.
What is the required tagging for the periodic inspection on a (PIT)?Must have a tag or equivalent system in place to demonstrate adequate maintenance and inspection records (including date, time, etc. from lifting document) for any service, repair or modification which affects the safe performance of the equipment must be maintained and be reasonably available to the operator and maintenance personnel at all times. All PITs deemed unfit for use by inspection must be taken out of service and repaired and inspected before being put back into service.
Are there any other required inspections for (PIT)?Yes. (PIT) also requires a daily (pre use) documented inspection.
Is lift planning required when operating a (PIT)?Yes, lift planning is required for all Lifting and Hoisting operations. At a minimum verify that the answers to SS FR 3.12.343 Twelve Questions for a Safe Lift are addressed in a Toolbox Talk.
When is it acceptable to lift with one fork with a (PIT)?Never.
What is an Aerial work Platform (AWP)?An Aerial Work Platform (AWP) is a lifting appliance used to lift personal such as; extendable boom platforms, aerial ladders, articulating boom platforms, vertical towers (scissor lifts) or a combination of any such devices.
What is the required tagging information for a periodic inspection on an AWP?Must have a tag or equivalent system in place to demonstrate adequate maintenance and inspection records (including date, time, etc. from lifting document) for any service, repair or modification which affects the safe performance of the equipment must be maintained and be reasonably available to the operator and maintenance personnel at all times.
